Zusammenfassung
Die Behandlung der Frakturen am proximalen Femurende spiegelt den Wandel und die historische
Entwicklung der Traumatologie. Meilensteine waren die Extensionsbehandlung, die Entwicklung
von Implantaten für Osteosynthese sowie für Hemiarthroplastiken oder Totalarthroplastiken

[11]. Mit dieser Entwicklung sind Namen wie Whitmann (Repositions-Retensions-Gipsverband),
Smith-Petersen (Lamellen-Nagel in Amerika und Lorenz Böhler in Europa), AO Gruppe
(130° Winkelplatte und DHS Nagel),eng verbunden. In der zweiten Hälfte des 20. Jahrhunderts
hat sich der Prothetische Gelenkersatz bei Schenkelhalsfrakturen durchgesetzt.
Wir haben zum Teil diesen Wandel mitgemacht in unserem Material und bei erfüllten
Voraussetzungen, wie der Möglichkeit zur Notfall-Operation (war nicht immer selbstverständlich),
krankengymnastischer stationärer und
poststationärer Behandlung, idealer familiärer Obhut (nur 5 % unserer Patienten kommen
aus Altersheimen), sind unsere Resultate gut bis ausgezeichnet. Die Mortalität konnte
sehr stark gesenkt werden (bei Bipolaren Endoprothesen und Totalprothesen unter 4
%). Ebenso niedrig waren die Komplikationen und somit auch die Behandlungskosten bei
streng ausgewälter Indikation der Therapie-Methode und exakter Auswahl der richtigen
Implantate nach Frakturart.
Treatment concept of prochanteric fractures in elderly patients
Summary
The treatment of proximal femur fractures mirror in a high degree the change and the
historical evolution of Traumatologie. Milestones in this evolution were the treatment
by traction, the development of implants for osteosynthesys and for partial or total
joint replacement and the advantage of early mobilization the elderly patients. With
this evolution affiliate the names Whitman (closed reduction- belay- and plaster spica
in abduction), Smith-Petersen nailing (in USA, Boehler in Europe), 130° angleplate
und DHS AO nailing, sliding nailing. In the second half of 20th century onset the
hemi- and totalarthroplasty.
We have this change and the historical evolution participate with our material and
if the premises correspond, like the possibility for emergency operation (no always
self evident), perioperative and postoperative physical therapy, ideally familiar
condition (only 5 % our patients are from nursing homes), are our results good to
unexceptionably.
Mortality manage heavily decreased incidence (in bipolars hemiarthroplastics and totalarthroplastics
under 4 %).
Similarly decreased was the complication rates and the cost effectiveness of the treatment
when strictly indication for treatment method and implants selected.
